Health Educator We seek a professional who is passionate and committed to making a difference in the lives of our patients for our outpatient program. The right person combines committment, openness, to training and a desire to work intensively to positively impact patients' present and future health. Position involves leading patient groups and working with individuals using a behavioral and educational approach, patient phone follow-up, willingness to model a healthy lifestyle and going the extra mile with patients to affect change.
